Rural Oregon Outreach & Telehealth Services
Amy Jordan, FNP
At Western Roots Healthcare, we are dedicated to providing accessible, evidence-based healthcare to our community. We aim to improve the health and wellness of rural Central Oregon through a variety of telehealth services.
We make high-quality healthcare easy and accessible for rural Central Oregon and beyond, no insurance required. Our telehealth platform lets you connect with a local, board-certified provider from the comfort of your home, with simple online payments and transparent fees.

I currently can see you for medical issues like:
With secure video visits, you will get prompt, compassionate treatment for everyday illnesses - no clinic trip required.
** Please note that Western Roots Healthcare does not prescribe controlled substances at this time, including opioid pain medications, benzodiazepines like Xanax, ADHD medications, or testosterone at this time. DEA licensing is currently pending - we will update availablity as soon as this changes. **
We’re currently working on becoming credentialed to accept Medicare, OHP, and major Oregon insurance plans to make it easier for you to access our care. While we complete this process, patients can still be seen and will receive a detailed invoice (superbill) that can be submitted to private insurance for possible reimbursement or application toward a deductible. Please note that reimbursement is not guaranteed and varies by individual insurance plan. Medicare/OHP regulations do not allow submission for reimbursement at this time.

A flat fee telehealth visit is a simple, all‑inclusive charge for your virtual appointment, with no extra surprise costs. For up to a 30‑minute visit, this typically includes a secure video consultation, review of your medical history, medications, current concerns, assessment and medical decision‑making, creation or adjustment of a treatment plan, and any necessary electronic prescriptions, laboratory or radiology orders.
Online payment information is input during scheduling of appointment and processed day of visit. We accept Visa, MC or HSA/FSA cards with a Visa/MC emblem.

I’m Amy Jordan, a board-certified Family Nurse Practitioner and founder of Western Roots Healthcare, a telehealth practice serving individuals across Oregon. My career in healthcare spans more than 25 years, beginning with a decade in the clinical laboratory followed by 15 years as an emergency department RN before becoming a Family Nurse Practitioner. This background gives me a detailed understanding of how your symptoms, your lived experience and overall health fit together.

I have a deep love for the Pacific Northwest and the people who call it home. When I am not seeing patients, you are likely to find me skiing in the mountains, digging in the dirt, hiking through sagebrush to get that elusive perfect photo, or spending time on the farm with my dogs while they chase the cows and chickens.

I created Western Roots to make it easier for patients in rural and underserved communities to access a provider that understands their challenges, takes time to listen, and offers clear, straightforward plans.

In our telehealth visits you can expect thorough attention to your story, careful review of your history, clear explanations in plain language, and a care plan that we build together. I believe in holistic care that considers your physical health, your environment, your routines, and what matters most to you. Availability and follow‑through are important to me. My goal is for you to feel you have a provider who knows you, is there when you need care, and is committed to you over time, whether you live in town or at the end of a dirt road.
Western Roots Healthcare plans to open a physical clinic in Terrebonne, Oregon in 2026. At that time, in‑person primary care will be available for chronic conditions such as diabetes, high blood pressure, high cholesterol, thyroid problems, and more, with personalized care plans, regular follow‑up visits, and ongoing monitoring to support long‑term health and prevention goals. The clinic will also offer focused care for midlife health, including perimenopause and menopause, hormone therapy, low testosterone, and medically supervised weight management.
