I’m Amy Jordan, a board-certified Family Nurse Practitioner and founder of Western Roots Healthcare, a telehealth practice serving individuals across Oregon. My career in healthcare spans more than 25 years, beginning with a decade in the clinical laboratory followed by 15 years as an emergency department RN before becoming a Family Nurs

In 2026, Western Roots Healthcare is preparing to open a physical clinic in Terrebonne, Oregon. At that time, I will be able to deliver comprehensive in-person primary care, with a special focus on midlife women’s health, perimenopause/menopause management, hormone replacement therapy, low testosterone, and medically managed weight loss. I will also provide comprehensive management for chronic health conditions, including diabetes, high blood pressure, high cholesterol, thyroid disorders, and more, with customized care plans, routine follow-up visits, and ongoing monitoring to support your long-term wellness and prevention goals.
As the practice grows to provide primary care, I plan to transition to a Direct Primary Care (DPC) model, where patients pay a simple, transparent membership fee for enhanced access to primary and preventive care, instead of relying on traditional insurance billing for office visits. This approach allows me to invest time in patient care instead of time-consuming paperwork. Additionally, I will be able to offer a wider range of non-insurance-covered treatments like IV hydration and vitamin therapy, cutting-edge peptide therapy, wellness optimization, health screenings, remote patient monitoring, minor procedures like cosmetic injections, cryotherapy, and rural health outreach clinics. My goal is to make advanced, patient-centered care accessible, affordable, and convenient for Central Oregon’s rural communities.

A flat fee telehealth visit is a simple, all‑inclusive charge for your virtual appointment, with no extra surprise costs. For up to a 30‑minute visit, this typically includes a secure video consultation, review of your medical history and current concerns, assessment and medical decision‑making, creation or adjustment of a treatment plan, and any necessary electronic prescriptions or lab orders.
Online payment information is input during scheduling of appointment and processed day of visit.
