I’m Amy Jordan, a board-certified Family Nurse Practitioner and founder of Western Roots Healthcare, a telehealth practice serving individuals across Oregon. My career in healthcare spans more than 25 years, beginning with a decade in the clinical laboratory followed by 15 years as an emergency department RN before becoming a Family Nurs

The connection to this land and to this way of life is part of why I have a strong passion for rural medicine and for bringing high‑quality care to people who often have the fewest options. I created Western Roots to make it easier for patients in rural and underserved communities to access a provider that understands their challenges, takes time to listen, and offers clear, straightforward plans.

Availability and follow‑through are important to me. My goal is for you to feel you have a provider who knows you, is there when you need care, and is committed to you over time, whether you live in town or at the end of a gravel road.

Telehealth Visits
I currently can see you for simple medical issues like:
- Cold, cough, sore throat, and sinus infections
- Allergies and minor skin rashes
- Urinary tract infections
- Headaches and migraines
- Nausea, vomiting and mild gastrointestinal symptoms
- Simple medication management (excluding controlled substances)
- Other medical issues that do no require a personal visit (feel free to ask me!)
With secure video visits, you will get prompt, compassionate treatment for everyday illnesses - no clinic trip required.
2026 Clinic Plans
In 2026, Western Roots Healthcare is preparing to open a physical clinic in Terrebonne, Oregon. At that time, I will be able to deliver comprehensive in-person primary care, with a special focus on midlife women’s health, perimenopause/menopause management, hormone replacement therapy, low testosterone, and medically managed weight loss. I will also provide comprehensive management for chronic health conditions, including diabetes, high blood pressure, high cholesterol, thyroid disorders, and more, with customized care plans, routine follow-up visits, and ongoing monitoring to support your long-term wellness and prevention goals.
Additional Services on the Horizon
As the practice grows to provide primary care, I plan to transition to a Direct Primary Care (DPC) model, where patients pay a simple, transparent membership fee for enhanced access to primary and preventive care, instead of relying on traditional insurance billing for office visits. This approach allows me to invest time in patient care instead of time-consuming paperwork. Additionally, I will be able to offer a wider range of non-insurance-covered treatments like IV hydration and vitamin therapy, cutting-edge peptide therapy, wellness optimization, health screenings, remote patient monitoring, minor procedures like cosmetic injections, cryotherapy, and rural health outreach clinics. My goal is to make advanced, patient-centered care accessible, affordable, and convenient for Central Oregon’s rural communities.
